The Outreach service offers patients and their families access to specialist support and nursing care at home 7 days a week and is available for patients on their return home from MSH. The service is free of charge and is funded
by the Friends of Michael Sobell House.
The Outreach Service works closely with the District Nurse, who
will remain the key worker, and with
the Macmillan Nurse and GP to provide the best support and nursing
care available.
A team of specialist support nurses provide the Outreach Service.
They are all experienced nurses who have worked at Michael Sobell
House and they will provide whatever care is needed, both physical
and supportive. This may be for short periods of time normally up to 1 or 2 hours a day.
After after 2 or 3 days hopefully the Outreach team will have helped the patient through the transition period from MSH to home enabling them to remain at home with the support of the District Nurse and other
community agencies.
However, if admission to Michael Sobell house is necessary, this
will be arranged after discussion with the team.
If the Service is required at a later stage, then another period
of nursing support can be arranged.
